A simple report is enough to start; no need for an A3 every time. We design tools that steer each problem toward a proportionate treatment, follow actions until effectiveness is proven, and turn what was learned into prevention for your other lines, workstations or products.
Examples of tailor-made solutions
- Reporting and qualification: a quick report, then a triage that assesses criticality and spots duplicates and recurrences.
- Proportionate resolution: simple treatment, A3 or 8D path depending on severity; facts, causes and evidence already entered are kept if the path changes along the way.
- Action backlog: immediate measures, corrective and preventive actions, with two distinct states: completed, then verified as effective (or reopened if the problem recurs).
- Risk register: assessments, control measures and matrix, reassessed over time, not only after an incident.

Your everyday challenges
Requiring an A3 for every report discourages reporting; structuring nothing loses the lessons from one incident to the next. Between the two, a common base (same facts, same causes, same evidence) lets you adjust the level of treatment without re-entry, and tell an action that was merely completed from one that is truly effective over time.
