Detect, qualify, analyse, act, verify, capture, prevent, monitor: a continuous loop rather than a one-way trip. A simple report is enough to start, the resolution path (simple, A3 or 8D) adapts to severity, and a verified lesson can be rolled out to other lines exposed to the same risk.
The problem
Between the binder of A3s that discourages reporting small problems and the non-conformity spreadsheet that never links an incident to risks or prevention, the company loses the loop: the same causes come back, closed actions were not always verified effective, and a lesson stays confined to the line where it was found.
The tool
A common base of facts, causes, evidence and actions, shared by every resolution path. Reporting always stays simple; qualification steers towards proportionate handling, never forcing an unnecessary A3 or 8D.
How it works
- Qualification: type, criticality (severity × frequency) and duplicates suggested from known reports.
- Resolution case: immediate measure, causes (hypothesis or confirmed), actions, with template changes (simple ↔ A3 ↔ 8D) that lose no data already entered.
- Backlog: each action follows a real cycle: to do, in progress, done, then verified effective or reopened if the problem comes back.
- Risk register: a control measure can be created straight from a risk, without waiting for an incident.
- Prevention: a check created from a lesson rolls out to the other stations or lines exposed to the same risk, and a non-compliant result creates a report automatically.
